State Application Process

We offer assistance in completion of State License Applications for the following:


Completion of the Department of Social Services/Community Care Licensing Division application for a facility license – commonly referred to as the Part A & B Application – can be an overwhelming and time-consuming process. With combined decades of experience in the legal field and elder care industry, DynamiCare’s principals are able to guide you through completion of both Part A licensing forms and the supporting documentation required for Part B.
